Make a complaint

Please use this form if you wish to make a formal complaint about the service you have received from SHAPES. Your complaint will be sent to the right department who will acknowledge it within 2 working days, followed by a written response within 21 days.

In your message please include as much details as possible regarding:

  • Which service you want to complain about.
  • What the issue is - including when it happened.
  • Whether you have already been in contact with anyone about this - if so, who and when were you in touch with them.
